Quinney Transitional Care is a Medicare-certified program that provides nursing care and rehabilitation services for patients who have been discharged from the hospital, but still require intensive therapy or skilled nursing services.

The patient's care is based on a comprehensive assessment and treatment plan developed by an interdisciplinary team of health care professionals who work closely with the patient and the patient's family to design a customized program of care.
